Full Time Position: Seeking a highly motivated and self directed individual to implement an intensive case management program with hospital patient population. This individual will be responsible for conducting outreach, education, enrollment, referral and linkage services to community services in the San Fernando Valley and Los Angeles County. Fulltime HR Generalist needed to perform a variety of HR functions.
