Full Time Position: Seeking a highly motivated and self directed individual to implement an intensive case management program with hospital patient population. This individual will be responsible for conducting outreach, education, enrollment, referral and linkage services to community services in the San Fernando Valley and Los Angeles County. Candidate should have at minimum a BA/BS level education (Masters level preferred), preferably in social work, public health, psychology or other health related field. Ideal candidate will be Bilingual (Spanish/English), possess minimum of 2 years experience in a hospital and/or healthcare setting, extensive knowledge of health and social services in San Fernando Valley, and experience case managing patient through multiple health systems.
